Our client is looking for a Registered Nurse to direct patient care from shift to shift to ensure an exceptional care experience. Primary accountabilities include care coordination, quality, and patient and family satisfaction. In partnership with the Unit Nurse Manager, the Patient Care Facilitator shares ongoing responsibilities for the outcomes of care for patients throughout their stay in a designated unit.
 
Registered Nurse - Patient Care Facilitator Responsibilities:
  • Makes appropriate patient assignments based on patients clinical status/care needs, staffing models and acuity during shift and for the next shift
  • Works with House Supervisors/staffing office in the anticipation of staffing levels needed in relationship to patient care needs, flow/volume, opening/closing of overflow units
  • Understands and follows the policy for staffing, scheduling and the OT/Low needs process
  • Utilizes the orientation binder and preceptor documentation tools to determine staffing needs for the current and next shift when residents and/or new hires are scheduled
  • Ensures Plans of Care are individualized/documented (i.e. anticoagulation, CHF, Diabetes, Restraint, etc.)
  • Resource for staff related to patient clinical concerns, assignments, acuity level and discharge planning
  • Responds to Code Blue/Rapid Responses/house wide emergency situations per policy
  • Participates in verbal de-escalation training and is a resource to the urgent needs of the unit, patients and staff
  • Rounds with MDs to support patient care needs/communicates Plan of Care with RN caring for patient
Registered Nurse - Patient Care Facilitator Requirements:
  • 3-5 years nursing experience in a hospital setting. Previous experience in a leadership role preferred
  • Previous experience and participation in care coordination and quality management activities preferred
  • Demonstrated ability to maintain composure in high stress situations; acts quickly in response to critical situations
  • Ability to be accountable for quality customer service and respond appropriately to a variety of personalities and situations
  • Must be able to read, write and speak effectively in English
  • Bachelor’s Degree in nursing preferred
  • Current license as a Registered Nurse
