Physician - On-Site Care Responsibilities:
  • Provide services, support, and care coordination to patients on a daily basis in our partnering communities
  • Visit individuals within all types of senior living communities
  • Build lasting relationships with your patients across their care continuum
  • Join a growing team with an excellent support staff
Physician - On-Site Care  Qualifications:
  • You have a strong portfolio as a Primary Care, Geriatrician, Hospitalist, or Internal Medicine doctor
  • You have a passion for serving the Senior Population
  • Licensed physician
  • Valid driver’s license and current auto insurance
